Office Hours & Appointments
Office hours are 8 a.m. to 5 p.m.,
Monday through Friday. Patients are seen by appointment
except in the case of emergencies. To avoid scheduling
problems we suggest that you call in advance as far
as possible to make your appointment.
New Patients should report at least
15 minutes prior to their appointment time for registration

Emergency Care
Accidents can and do happen! If you should need emergency
orthopedic care after normal office hours, weekends
or holidays. please call our office number 903-592-6811.
One of our physicians is always on call to provide
you with immediate medical care. You may also go to
the emergency room at East Texas Medical Center or
Trinity Mother Francis Hospital, where one of our physicians
will be contacted by the hospital.
Insurance
Our physicians are providers for most major insurers,
including HMO's and PPO's. HMO insurers may require
authorization from your primary doctor prior to seeing
a specialist. You must get that authorization prior
to your appointment with us. We also accept assignments
for Medicare. Workers compensation is gladly accepted
when properly verified in advance of the appointment.
Telephone Calls
If you call to speak with your physician or the nurse,
please understand that circumstances may not allow
this at the time of your call. Please leave your name
and number on the nurses' voice mail so that she may
return your call. If your call is of an urgent nature,
please let the operator know.

Counselor at 903-597-2923.
Prescriptions
For prescription refills, please contact your pharmacist
during regular office hours. The pharmacist will contact
our office for authorization of your refill.
Important Information
CASH PATIENTS It is our understanding
that you will be receiving services from Azalea Orthopedic
on a self-pay basis. The charge for the first visit
with the doctor will be a minimum of $150. Please
come prepared to pay for all charges from the first
visit. Since we are specialists, many times more
tests or treatment is needed. At the time these services
are provided, payment is due in full. If you have
any questions, please discuss them with our staff.
HMO INSURANCE If your insurance
company is an "HMO" and requires you to
have a referral from your primary care physician
(PCP), PLEASE call them and make sure they have sent
it to us BEFORE your appointment, or you will be
required to pay all charges at the time of your visit.
You also need to know what hospital, lab and pathology
facility you are required to use.
PPO INSURANCE If your insurance
is a "PPO," please check with your insurance
company to see if the doctor you will be seeing at
Azalea is in your network PRIOR TO YOUR APPOINTMENT,
or you will be required to pay all charges at the
time of your visit. You will also need to know what
hospital, lab and pathology facility you are required
to use.
MEDICARE PATIENTS If you have chosen a plan other
than standard Medicare, check with your insurance
company prior to your visit to determine if the doctor
is in the plan. If any referral is necessary, it
is your responsibility to obtain it prior to the
visit. You will also need to know what hospital,
lab and pathology facility you are required to use.
This is also necessary for all supplements to Medicare.
You must know what kind of insurance you have. Check
with your insurance carrier if you are not sure.
All patients are required to pay their deductible
and copay or percentage amounts at the time of visit.
